A d\u00eda de hoxe, anticipar a evoluci\u00f3n da enfermidade en cada persoa segue a ser un dos principais desaf\u00edos cl\u00ednicos. As clasificaci\u00f3ns actuais bas\u00e9anse maioritariamente nos <strong>s\u00edntomas cl\u00ednicos<\/strong>, que non sempre reflicten os <strong>mecanismos biol\u00f3xicos reais<\/strong> responsables do dano no sistema nervioso central.<\/p>\n\n\n\n<p>Esta investigaci\u00f3n demostra que, mediante a combinaci\u00f3n dun <strong>biomarcador sangu\u00edneo<\/strong> e <strong>imaxes cerebrais est\u00e1ndar<\/strong>, analizadas cun modelo avanzado de IA, \u00e9 posible identificar <strong>formas de EM claramente diferenciadas dende o punto de vista biol\u00f3xico<\/strong>.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Como se desenvolveu o estudo<\/h3>\n\n\n\n<p>O traballo estoudou e empregou dous elementos:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Niveis sangu\u00edneos da cadea lixeira de neurofilamentos s\u00e9ricos (sNfL)<\/strong>, un marcador fiable de dano neuronal.<\/li>\n\n\n\n<li><strong>Resonancias magn\u00e9ticas cerebrais<\/strong>, que permiten visualizar a propagaci\u00f3n da enfermidade e os cambios estruturais no cerebro.<\/li>\n<\/ul>\n\n\n\n<p>Estes datos foron analizados mediante un modelo de aprendizaxe autom\u00e1tica desenvolvido polo UCL, denominado <strong>SuStaIn (Subtype and Stage Inference)<\/strong>, dese\u00f1ado para identificar subtipos e estadios da EM base\u00e1ndose en datos biol\u00f3xicos obxectivos.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">A mostra analizada<\/h3>\n\n\n\n<p>A investigaci\u00f3n avaliou datos de <strong>634 persoas diagnosticadas<\/strong>, procedentes de d\u00faas series de ensaios cl\u00ednicos. En cada caso:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Realizouse unha <strong>an\u00e1lise de sangue<\/strong> para medir os niveis de sNfL.<\/li>\n\n\n\n<li>Avali\u00e1ronse mediante <strong>resonancia magn\u00e9tica<\/strong> os cambios estruturais cerebrais e a aparici\u00f3n de lesi\u00f3ns.<\/li>\n\n\n\n<li>O modelo SuStaIn integrou toda esta informaci\u00f3n para identificar <strong>patr\u00f3ns biol\u00f3xicos espec\u00edficos<\/strong>, prescindindo dos s\u00edntomas cl\u00ednicos como criterio principal.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Dous tipos biol\u00f3xicos de EM<\/h3>\n\n\n\n<p>O estudo identificou dous subtipos biol\u00f3xicos de EM claramente diferenciados:<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Tipo sNfL temper\u00e1n<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Niveis elevados de cadea lixeira de neurofilamentos en sangue dende fases iniciais.<\/li>\n\n\n\n<li>Dano temper\u00e1n no corpo caloso.<\/li>\n\n\n\n<li>Formaci\u00f3n r\u00e1pida de lesi\u00f3ns cerebrais.<\/li>\n<\/ul>\n\n\n\n<p>Este perfil corresponde a unha <strong>forma m\u00e1is agresiva de Esclerose M\u00faltiple<\/strong>, cun maior risco de progresi\u00f3n precoz.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Tipo sNfL tard\u00edo<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>A atrofia cerebral en \u00e1reas como a cortiza l\u00edmbica e a substancia gris profunda aparece antes do aumento dos biomarcadores sangu\u00edneos.<\/li>\n\n\n\n<li>A progresi\u00f3n da enfermidade \u00e9 m\u00e1is lenta.<\/li>\n<\/ul>\n\n\n\n<p>Os resultados deste estudo foron publicados na revista cient\u00edfica <strong>Brain<\/strong>, referente internacional en neurolox\u00eda.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Implicaci\u00f3ns cl\u00ednicas e conclusi\u00f3ns<\/h3>\n\n\n\n<p>Este descubrimento permite ao persoal sanitario:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Predicir con maior precisi\u00f3n<\/strong> que persoas con EM presentan m\u00e1is risco de desenvolver lesi\u00f3ns cerebrais.<\/li>\n\n\n\n<li><strong>Anticipar a progresi\u00f3n da discapacidade<\/strong>, incluso antes de que aparezan signos cl\u00ednicos evidentes.<\/li>\n\n\n\n<li><strong>Axustar o seguimento e o tratamento<\/strong> de forma m\u00e1is temper\u00e1 e personalizada.<\/li>\n<\/ul>\n\n\n\n<p>Identificar a biolox\u00eda da EM antes do deterioro cl\u00ednico abre a porta a <strong>terapias m\u00e1is espec\u00edficas<\/strong>, dirixidas aos mecanismos subxacentes de cada persoa con EM.<\/p>\n\n\n\n<p>Tal e como sinala o Dr. Arman Eshaghi, autor principal do estudo e fundador de Queen Square Analytics:<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>\u201cA Esclerose M\u00faltiple non \u00e9 unha soa enfermidade.
